I acknowledge that I:

• am over the age of 18 years OR I am the parent/guardian of a person who is under 18 years and have the legal authority to give consent;
• understand that I can withdraw this authorisation at any time and this it my responsibility to inform afs
• understand that once information has been published on the Internet, afs has no control over its subsequent use and disclosure
• understand the agreed purpose of publication of images or recordings of me, or my artworks, or quotes or testimonials from me, including possible use on the Internet;
• understand that afs does not guarantee that any images, recordings, quotes or testimonials will necessarily be used;
• understand that copyright in the images or recordings, or documents containing the published images, quotes or testimonials will vest in afs
• consent to the publication, by afs, of images or recordings of me, of my artworks, or of quotes or testimonials from me for the agreed purpose with any reasonable retouching or alteration (including extracts, cut-downs or stills).
